The article presents the electronic substitution diary smart medication, along with an analysis of the real-time data collected with it. Since 2012, smart medication has been used by 663 patients at 30 German hemophilia treatment centers (HTCs). Data from 9 HTCs were merged for the following analysis. Four groups were formed according to the mean annual factor consumption and the number of joint bleeds. The largest group (A, 42%) was the one with below-average factor consumption and fewer than 2.25 joint bleeds per year. The second-largest group (B, 32%) showed similarly low bleeding rates with above-average factor consumption. A reduction in factor consumption might be possible for some of these patients. In contrast, other patients with frequent joint bleeds combined with low factor consumption (D, 13%) could benefit from an increase in dose. Patients with frequent joint bleeds despite above-average factor consumption (C, 13%) require intensified treatment beyond a dose adjustment, such as pharmacokinetically adapted dosing or orthopedic measures. With the help of smart medication, patients who require a change to their ongoing treatment can be identified long before their next visit to the HTC. The constantly growing data pool also allows continuous analysis of real-time data and thus an optimization of the available resources.
